Jennifer Lloyd is a seasoned nurse leader with over 20 years of experience in designing and leading innovative Care Management programs across integrated health systems and managed care plans. Known for her commitment to advancing patient-centered care, Jennifer has consistently worked to improve healthcare access and outcomes for vulnerable populations.
In 2000, Jennifer was honored with the Maryland State Innovation in Healthcare Award for her pioneering Care Management Without Walls initiative, which brought hospital care managers into community and primary care settings to support underserved populations in Southern Maryland.
In 2012, Jennifer and her wife relocated to California, where she served as Vice President of Ambulatory Care at Dignity Health Medical Foundation, leading the integration of ambulatory care managers into primary care offices and hospitals to support seamless care transitions. Most recently, Jennifer served as Senior Vice President of Population Health and Clinical Operations at Health Net, where she led the development of a Utilization Management program in support of CalAIM, removing barriers to care and expanding access to Enhanced Care Management services and Community Supports for California’s most vulnerable members.
Jennifer is proud to help support the hospital campus redesign and the organizations that will be promoting health equity, support homeless and low-income populations, and serve the most vulnerable. Jennifer is a registered nurse with a Bachelor’s in Business Administration and a Master’s in Healthcare Administration. She is passionate about redesigning the healthcare system to achieve health equity and uplift communities in need.
